Breast Augmentation: Silicone Implants vs. Fat Grafting Explained

Summary:

Dr. Anand Kumar Nagwani, a Plastic & Cosmetic Surgeon, explains the two main methods of breast augmentation: silicone implants and autologous fat grafting. Implants involve placing a silicone prosthesis in the breast, while fat grafting uses the patient’s own fat, harvested and processed through liposuction, and then injected into the breast tissue. Both methods are safe and effective, but each comes with specific considerations. Implants are durable and widely used, while fat grafting may result in variable outcomes due to fat absorption—especially in smokers or alcohol consumers.

Dr. Nagwani emphasizes that breast augmentation is suitable for women of various age groups, provided they are medically fit. He addresses common concerns such as ideal age, recovery time, complications, and cost. Typically, hospital stay is short (3–5 hours), and most patients can return to work within a few days. He also highlights the importance of pre-surgery health checks and advises avoiding smoking before and after surgery.

At his clinic, patients receive complete guidance and transparency, with both augmentation options available to suit individual needs and preferences.

Conversation

Namaskar, I am Dr. Anand Kumar Nagwani, a Plastic & Cosmetic Surgeon and Hair Transplant Specialist.
Apart from this, I also treat various cosmetic surgery concerns.Breast augmentation has become increasingly popular as a cosmetic procedure in recent years. Breast augmentation means boobs enlargement—this is the term used in everyday language, but the actual medical term is breast augmentation.

This can be done either by using implants (prosthesis) or through your own fat. We process and refine your body’s fat and inject it back. So, there are two practical methods—

  1. Using prosthesis/implants
  2. Using fat

So now we’ve understood these two ways.

People usually have many questions:
– Who can undergo this surgery?

-What is the appropriate age to consider this surgery?”
-What are the possible complications?
– What is the cost involved?
– What is the expected healing time after the procedure?

 – How many days do we need to stay in the hospital?

These kinds of questions are very common and justified. Because when someone thinks about undergoing breast augmentation or boobs enlargement, such thoughts naturally arise in their mind.

The first question is always—how is it done?
As I mentioned, there are two methods. Let me show you a prosthesis first. This is a dummy prosthesis.

Look at this—this is a dummy prosthesis, and we call it a breast implant.
Now see—I’m pressing and squeezing it in front of you. I’m applying a good amount of pressure, and still nothing is happening to it. If I press it from the middle, it turns into a dumbbell shape. So basically, it’s very strong.

Now about how it feels—the touch feels normal.
I’ve shown you its strength. One thing to understand: it can’t withstand sharp objects.. Maybe 20–25 years from now, it might be possible, but currently, it’s not.

These are made of silicone, and in today’s date, we can call them modern. This one is from Motiva company, which we actually use. Why do we use it? I’ll tell you that too.

Right now, smooth breast implants are considered the best. I’ve been doing breast implant surgeries for the past 18 years. Over time, I’ve seen a lot of changes in expectations and also in the number of cases. Earlier, people were hesitant, but now it’s very easily available and commonly done.

Over this long period, I have seen many breast implant patients and their experiences, and I’ll share that too. But before that, let me show you something important.

These implants come with specific directions for use, and you also get a guideline booklet. It contains information about projection, diameter, etc. Everything is explained in detail.

Just like in America and Europe, where no one proceeds to surgery without knowing everything, we also follow the same in our clinic. When a patient comes to us considering breast augmentation, we explain everything clearly, which is why I’m discussing everything in this video too.

Now let’s talk about who can undergo this surgery?
What age group?
Who are the ideal candidates?

Usually, it is women in the reproductive age group. But that doesn’t mean it is limited to them—even beyond this age, women have come to me for breast augmentation, and they are very happy with the results.

The youngest patient I operated on was 17 years old, and that too with guardian consent. Even at 14 years, one can give self-consent. However, in India, guardian consent is essential, even for patients above 18.

18+ patients still involve guardians, and that’s absolutely fine—we welcome it.
It’s not just about being 18—the patient should also be mentally capable of making a decision.

What about upper age?
There is no fixed upper age limit. Women even in their 50s or 60s can opt for it. It’s not like surgery is only meant for reproductive-aged women.

If you wish to undergo surgery for cosmetic purposes, and are medically fit, then you can go ahead.

Now, when I say medically fit, I mean:
– Blood pressure should be normal
– No heart problems
– Blood sugar should be normal
– Liver functions should be normal

When I keep saying “normal,” it doesn’t mean that people with diabetes, high BP, or heart issues cannot undergo surgery. They can, provided their condition is controlled with medication.

I have performed breast implant surgeries on many diabetic and hypertensive patients, and they’ve had successful outcomes.

One special point I always emphasize:
“For safer outcomes and improved results, we recommend discontinuing smoking prior to your surgery and maintaining that habit-free period for a minimum of six months afterward”

Because smoking can cause increased bleeding and has other drawbacks. Now let’s talk about some other important points:
Any prosthesis—whether for breast, orthopedic surgery, heart stents, or eye lenses—comes with a common risk of infection or bleeding. But these are very rare today, thankfully.

Such complications usually arise only if the patient is taking blood-thinning medications.
So, if you’re getting any prosthesis in your body, infection prevention is the most important thing.

Otherwise, in general, these risks are minimal.

Now, some minor issues should also be explained.
The right and left breasts—like any two sides of the human body—are never exactly the same. There are always minor differences, especially in females.

You’ll often notice that the size, mount, nipple height, projection, and angulation of both breasts are different.
These differences are natural in all human beings, and we must plan surgeries while accounting for them.

If the difference is minor, it doesn’t matter much.
If the difference is major, then sometimes we need to adjust the implant size accordingly.

For example, we may use a 350cc implant on one side and a 300cc on the other.

Some patients might feel changes in sensation—this usually resolves over time.
In rare cases where it doesn’t, medicines can help, and even then it’s very rare that it remains permanent.

Also, one thing I’ve noticed, and it’s even been discussed in medical conferences, and now mentioned in implant manuals, is that—
There have been very rare reported cases of cancer associated with breast implants.
Although very rare, it is still important for you to be informed.

That’s why I’ve shared and discussed all this with you.

Let’s discuss the typical duration of your hospital stay following the surgery

You typically stay for less than half a day.

You arrive in the morning, undergo surgery, and within 3–4 hours, you feel normal.
Within 5 hours, you start eating and walking.

And once you’re up and about, we usually say:
“If you’re feeling fit, go home.”

Next question patients often have is:
How soon can you get back to your job or daily routine after the procedure?

Usually, a 3–4 day leave is enough.
Taking a week off would be ideal for a smoother recovery.

Some points to always remember post-surgery:
–Avoid swimming during the initial recovery period
– Don’t lift heavy weights

We explain all of these postoperative instructions clearly at the time of consultation too.

Apart from this, breast augmentation can also be done using autologous fat grafting if the patient is willing. In such cases, we harvest fat from the patient’s own body, process it using liposuction, and then inject it into the breast tissue to achieve augmentation.

Some people are understandably apprehensive about this method. The two main concerns are fat absorption and the rare risk of fat embolism. However, since it is your own body’s fat, it is generally considered safe and biocompatible.

Sometimes, the expected results are not achieved—especially in smokers, as they tend to experience higher fat absorption rates, meaning some of the injected fat may not survive. A similar issue can be seen in individuals who consume alcohol frequently.

That said, fat grafting is not a bad option—it’s simply another method. The final decision is entirely up to the patient

At our center, most patients tend to opt for silicone breast implants, but we offer both options and let patients choose what best suits them.

Namaskar.
