About the Condition

No — gynecomastia is not the same as excess chest fat from being overweight, though the two can coexist. True gynecomastia is the enlargement of the glandular breast tissue in men, caused by a hormonal imbalance between oestrogen and testosterone. It presents as a firm, rubbery lump directly beneath the nipple-areola complex.

Pseudo-gynecomastia, by contrast, is purely fatty tissue without a glandular component — more common in overweight men. Many patients have a combination of both. A physical examination by Dr. Nagwani during consultation will determine exactly what you have and what treatment is appropriate.

In adolescent boys, gynecomastia caused by puberty-related hormonal fluctuation often resolves on its own within 1–2 years. However, in adult men, established gynecomastia with a glandular component does not resolve without surgery. The glandular tissue is permanent once formed.

Medications have been tried (tamoxifen, raloxifene) but their use is off-label and results are inconsistent. They do not work on established fibrous glandular tissue. For adult men with gynecomastia that has persisted beyond 2 years, surgery is the only reliable solution.

This is one of the most common and frustrating situations. Exercise builds the pectoral muscle beneath the breast tissue — but it cannot remove glandular tissue, which sits on top of the muscle. In fact, as your chest muscles grow, the glandular tissue can appear more prominent, not less.

No amount of cardio, bench press, or calorie restriction will eliminate glandular breast tissue. Surgery is the only intervention that physically removes it. This is biology, not a training failure.

About the Surgery

Gynecomastia is graded from I to IV based on the amount of tissue present and the degree of skin excess. Most patients presenting at Calcutta Cosmo Aid fall into Grade I–III and are good surgical candidates. Grade IV cases with significant skin excess may require additional skin excision.

Candidacy is assessed at consultation and considers your grade of gynecomastia, overall health, age, skin elasticity, and your goals. There is no way to determine candidacy without a physical examination — online photos and videos are no substitute.

In most cases, scarring is minimal and well-concealed:

  • Liposuction access ports are 3–4mm and heal near-invisibly
  • Glandular excision incisions follow the lower edge of the areola, where the colour contrast naturally hides the scar line
  • With proper scar care (silicone gel, sun protection), most scars fade to fine lines by 6–12 months

Patients with a personal history of keloid or hypertrophic scarring are assessed individually. Dr. Nagwani will discuss your specific scar risk at consultation.

During surgery: you feel nothing. You are under general or local anaesthesia with sedation.

After surgery: the chest feels tight, tender, and sore — similar to a heavy gym session. Most patients describe it as manageable discomfort rather than severe pain. Prescribed pain medication handles it effectively. The first 2–3 days are the most uncomfortable; most patients are moving around comfortably by day 4–5.

  • Desk job / office work: 3–5 days
  • Light physical work: 1–2 weeks
  • Heavy physical labour / gym: 3–4 weeks

Most patients return to normal daily activity within a week. You will need someone to accompany you home on surgery day and ideally stay with you for the first 24 hours.

About Results & Recovery

Results appear gradually as swelling subsides:

  • Week 1–2: Significant swelling — do not judge results at this stage
  • Month 1: About 40% of final result visible; shape beginning to emerge
  • Month 3: 80–90% of final result visible — the transformation most patients have waited for
  • Month 6–12: Complete, permanent final result

Patience is essential. Biology determines the timeline — not the quality of surgery.

Removed glandular tissue does not regrow. The results of surgery are permanent under normal circumstances. However, recurrence is possible if the underlying cause is not addressed:

  • Anabolic steroid or recreational drug use can cause regrowth in remaining tissue
  • Certain medications (anti-androgens, some heart medications) can stimulate breast tissue
  • Significant weight gain can cause fatty enlargement

Men who maintain a stable weight and avoid gynecomastia-causing substances will typically retain their results for life.

  • Light walking: From day 3–4
  • Lower body exercise (no chest strain): From week 2–3
  • Full gym including chest exercises: From week 4–6, as cleared by Dr. Nagwani at your review

Returning to exercise too early risks seroma formation and compromises the result. Follow the timeline given at your post-operative review.
